Questionnaire (SET411)

Use this page to create, find, and edit questions for the Application. When you navigate to this page, you will see a Search Form, as shown below:

 

 

We'll return to this Search Form in a moment. For now, click Add New to open the window for creating a new question.

 

Create a New Question

When you create a new question and set it to "Active", it will appear on all Applications. The window for creating a new question is shown below:

 

 

Select a School Year and Question Type from the drop-down menus provided, and enter a Sort Order in the field provided. Use the check-boxes provided to set the question as Required and/or Active, or to set the Sync status.

 

Enter the question in the field provided, and select (if the Question Type is "Yes / No") or enter (if the Question Type is "Text Box") a Default Value that will be used if no answer is provided.

 

Click Save to create the question; click Cancel to close the window without saving.

 

Important! Fields marked with * are required; you will not be able to continue until all required fields have been filled.

 

Field Name Description
School Year The academic year to which the item is relevant.
Question Type The type of question, such as “Combo Box” or “Text Box”.
Sort Order The sorting priority for the item. You may only use numeric values when filling this field.
Question The question as it will appear on the Application.
Active Check this box to make the item active, or leave it unchecked to make the item inactive.
Sync Designates whether or not the selected question will synchronize with EZMealApp.
Required Designates the selected question as Required; the user will not be able to proceed forward in the application if the selected question has not been answered.
Default Value Enter or select a default value that will be entered if the user does not provide an answer or choose an option for the selected question.

View an Existing Question

Now, let's return to the Search Form.

 

 

Use the fields provided to narrow your search for an existing question. You can also check the Display Inactive check-box to display questions that are inactive, or leave the box unchecked to only display Active results. Click Search to generate a list of results, as shown below:

 

 

Note: If your search terms are too broad, the search form will generate a list of the one-hundred most relevant items.

 

You can sort the results by Question, Question Type, School Year, Sort Order, or whether the question is Active or Inactive by clicking the headers at the top of each column. You can also Edit a question by clicking the icon in the first column of each row.

 

Edit a Question

Clicking the Edit icon opens a new window that looks like the Add New Question window. Here you can make any changes to the question's description, Question Type, School Year, and Sort Order. You can also make the question Active or Inactive by checking or unchecking the box provided. Once you have made the necessary changes, click Save to keep the changes to the question and close the window. Click Cancel to close the window without saving.
